Dear Dr. Menard,

Ethan L* is a 21-year-old female-to-male transgendered patient who first consulted with me on 5/5/03 in order to receive a recommendation to undergo mastectomy surgery as part of his gender transition process. I have met with Ethan twice, consulted with Ethan's previous therapist V* N*, B.S., who had met with Ethan for 14 sessions, and carefully reviewed the documentation prepared by Ms. N* in support of Ethan's hormone therapy, which he began on 5/15/03.

Ethan meets the diagnostic criteria for Gender Identity Disorder. He has been living successfully as a male for over one year, and has developed a positive social support network among his friends and family. He has responded positively to hormonal therapy. His records indicate that he is physically and emotionally healthy, with no evidence of psychopathology or impaired judgement. He is aware of the benefits and potential risks of surgery, and feels prepared to undertake this important step in his gender transition process.

In summary, after reviewing the evidence, I feel comfortable in recommending Ethan L* for surgery. He has met all of the eligibility and readiness criteria outlined in the Standards of Care for the treatment of transgendered individuals.
